Ask most LA practice owners who their competition is and they will name the four other dentists on the same boulevard. Ask Google, and you get a different answer.
Google decides which practices to show a patient based partly on how far that patient would realistically travel. In Los Angeles County, “far” is not measured in miles. It is measured in minutes on the 405 at 5:40pm. A practice 3.2 miles away across the freeway can be functionally further from a patient than one 6 miles away on the same side of it.
Almost every dental SEO campaign in LA is built on a mileage radius. That is the first thing we throw out.
Your Real Service Radius Is Smaller Than Your Map

LA patients do not behave like patients in Phoenix or Charlotte. Routine hygiene appointments are booked within a tight drive-time band — often ten to fifteen minutes because nobody schedules a 45-minute crawl for a cleaning twice a year.
That has two consequences most agencies miss:
Your general and hygiene keywords are hyper-local. You are not competing with all of Los Angeles for dentist near me. You are competing with the practices inside your patient’s tolerable drive. Winning those searches is a Google Business Profile and proximity problem, and it is more winnable than it looks.
Your high-value cosmetic and surgical keywords are not local at all. A patient will drive 40 minutes for a $28,000 full-arch case or a veneers consultation with a specific dentist. Those searches are county-wide and are won with content, authority and reviews not proximity.
Treating those two groups the same way is why so many LA campaigns stall. They need different pages, different targeting, and different success metrics. This split shapes everything in how we structure a dental SEO engagement.
LA County Is Seven Search Markets Wearing One Name

The county has 88 incorporated cities and roughly ten million people, but for search purposes it behaves as a handful of distinct markets that barely overlap in results.
The Westside — Beverly Hills, Santa Monica, Brentwood, Culver City, West Hollywood. The most saturated cosmetic market in the country. Veneers, smile design, and full-mouth reconstruction. Highest case values, highest content bar, longest timeline to rank.
The San Fernando Valley — Sherman Oaks, Encino, Studio City, Woodland Hills, Burbank, Van Nuys. Family and general dentistry volume, strong implant demand, meaningful bilingual search in the east Valley.
The San Gabriel Valley — Pasadena, Arcadia, Alhambra, Monterey Park, San Gabriel. Large Chinese-speaking and Spanish-speaking populations. Practices that signal language capability correctly pull patients from well outside their radius.
East LA and Southeast — Boyle Heights, Huntington Park, Bell Gardens, Downey, Whittier. Majority Spanish-language search intent. Almost entirely ignored by English-only SEO strategies.
The South Bay — Torrance, Manhattan Beach, Redondo Beach, El Segundo, Palos Verdes. Affluent, review-driven, heavy Invisalign and cosmetic demand.
Long Beach and the Harbor — Its own media and search market. Long Beach results rarely share anything with central LA.
The Foothills and North County — Glendale, La Cañada, Santa Clarita. Family practices, orthodontics, growing implant demand.
A “Los Angeles Dentist” page tries to speak to all seven and ranks in none. City-level pages, built with actual local detail rather than a find-and-replace on the city name, are what earn positions here.
The Paid Search Math Pushing LA Practices Toward Organic

Dental is already one of the most expensive verticals in Google Ads. In LA County, competition from DSOs and corporate groups has pushed clicks on implant and cosmetic terms into a range where a single booked consultation can cost several hundred dollars in ad spend before anyone sits in a chair.
That works if you close a high percentage of high-value cases. For most independent practices it is thin, and it stops entirely the day the card is declined.
The practical model we see working: run paid on your highest-margin procedures for immediate flow, build organic and map visibility underneath it, then move budget as organic bookings grow. The page you publish this quarter is still producing consultations in three years. The ad is not.
Spanish-Language Search Is the Largest Unclaimed Channel in LA Dental

Nearly half of LA County residents speak Spanish at home. A large share of dental searches happen in Spanish — dentista cerca de mí, precio de implantes dentales, dentista de emergencia, carillas dentales precio — and the competition for those terms is a fraction of the English equivalent.
Most agencies handle this by running the English site through a translation plugin. Google treats that roughly the way a patient would treat it: as something written by someone who does not actually speak the language.
Doing it properly means natively written Spanish treatment pages with correct hreflang implementation, Spanish-language attributes and services listed on the Google Business Profile, Spanish review responses written by a person, and staff language capability stated plainly on the site. If you have Spanish-speaking team members and are not signalling it, you are invisible to a substantial slice of high-intent local demand sitting in your own service area.
Where an Independent Practice Beats a DSO

Corporate groups outspend you. They do not outrank you automatically, and in dental search they have three structural weaknesses.
No named clinician. Google evaluates health content against a stricter standard, and pages with a real, credentialed, identifiable dentist behind them perform better. DSO content is usually authored by nobody.
Generic location pages. Group sites replicate one template across forty offices. Genuine local content a page that references actual neighbourhoods, parking, transit, and the specific cases that practice takes — outperforms it consistently.
Slow reviews. Corporate review responses are templated or absent. A practice owner replying personally within 48 hours moves conversion in a way no budget replicates. Patients read those replies before they call. You can see how this plays out for practices we work with on our results page.
Cosmetic Case Value Changes the Content Strategy

In markets where veneers and full-arch dominate, content transparency is the competitive edge. LA patients comparison-shop cosmetic dentistry harder than patients almost anywhere.
Practically, that means publishing genuine cost ranges rather than “call for pricing,” documenting recovery honestly including the parts patients dislike, explaining financing before they ask, and maintaining a before-and-after gallery with properly obtained written consent and no retouching. Add Dentist and FAQ schema so those answers can be pulled into Google’s AI Overviews, and name the licensed dentist who reviewed the page.
The practice that answers the cost question wins the search from the four practices that refuse to.

What We Will Not Promise
We will not guarantee a number one ranking. Nobody outside Google controls the index, and Google’s own guidelines list guaranteed rankings as a warning sign. We will not promise page one in thirty days in a market this competitive in LA, general and hygiene searches typically move in 60 to 90 days, while veneers, cosmetic and full-arch terms realistically take nine to twelve months.
What we do commit to: a defined scope each month, a baseline you can independently verify, full ownership of everything we build, and 30 days’ notice to walk away.
